Retina Injection and Buy-and-Bill Drug Billing
Intravitreal injections carry some of the highest dollar amounts in ophthalmology, and some of the smallest margins for error. LMC manages the drug, the procedure, and the authorization as one connected workflow.
Where revenue leaks
Buy-and-bill puts the practice's money at risk first.
Drug Units and NDC Reporting
Incorrect billing units or missing NDC information turn an expensive drug into an unpaid claim.
Wastage Modifiers
Single-dose drugs require the JW modifier for discarded amounts or the JZ modifier when nothing is discarded on Medicare claims.
Prior Authorization and Step Therapy
Payer drug policies change often, and an expired or missing authorization is discovered after the drug is administered.
Underpayments Hiding in Posting
Drug reimbursement below acquisition cost goes unnoticed without payment-level reconciliation.
How LMC helps
Retina billing, managed end to end.
Authorization Tracking
Drug coverage, step therapy, and authorization status verified before the injection.
Clean Drug Claims
Codes, units, NDC data, laterality, and wastage modifiers checked on every claim.
Payment Reconciliation
Drug payments reviewed against expected reimbursement, with underpayments pursued.
FAQ
Retina billing questions.
What is the difference between the JW and JZ modifiers?
For single-dose drugs on Medicare Part B claims, JW reports the amount of drug discarded, and JZ attests that no drug was discarded. One of the two is expected on these claims.
Do all payers require NDC numbers on drug claims?
Requirements vary by payer. Many commercial and Medicaid plans require NDC information, so LMC builds payer-specific rules into claim review.
Can you help with biosimilar transitions?
Yes. Payer preference for biosimilars changes authorization, coding, and reimbursement. LMC tracks those rules so the practice is not surprised at payment.
Make sure every injection gets paid correctly.
