When you submit our revenue review request form, we collect the information you provide: your name, work email, phone number, practice name, city and state, number of providers, practice management system, billing model, and any message you include.

Protected health information
This website is not intended to receive patient information or protected health information (PHI). Please do not submit PHI through our forms or by email to our general address. If PHI is received through the website, we will delete it. Client PHI is handled only under a business associate agreement and through secure channels.
